Why I believe Fluoxetine / Prozac is the best SSRI overall : depressionregimens
After discovering Moclobemide was causing a bizarre increase in daytime sleepiness/ fatigue, and also after realising that it isn't available in a country I plan to move to, I've recently decided to wean off it, and then had a very painful time finding a suitable replacement antidepressant. Having finally landed on Fluoxetine, it has reminded me why Fluoxetine may be the better option over other SSRIs when it comes to treating severe depression, based on studies I've read:
P-Glycoprotein. This is definitely the most important factor when choosing SSRIs. P-Glycoprotein is basically an enzyme that limits the ability of several drugs/antidepressants from entering the brain. The most notorious for being affected is Citalopram/Escitalopram, though a range of other antidepressants are also affected. For myself, even when on high dose Escitalopram, my depression deteriorated so bad that suicide suddenly became 'an option' again. When I got my genes tested: surprise surprise, I found that I have a hyperactive variant of P-Glycoprotein. As it turns out, Fluoxetine is one of the few antidepressants that is not affected by it, so even if you have a highly active version of P-Glycoprotein, Fluoxetine will bypass it anyway. Other drugs that have found to be unaffected include Mirtazapine, Moclobemide, Bupropion and Duloxetine (though there may be others). Interesting note: there have been studies that show that if you combine Escitalopram with a potent P-Glycoprotein inhibitor like Verapamil, its concentration in the brain increases significantly - but this is merely academic at this stage.
Behind Fluvoxamine, Fluoxetine is the 2nd most potent Sigma-1 agonist of all antidepressants. Activation of Sigma-1 receptor itself has shown to cause antidepressant & anxiolytic effects. Note: Sertraline also has a high affinity for Sigma-1 receptors, but is considered to be an antagonist rather than an agonist -> A study found that the positive effects of neurite outgrowth induced by Fluoxetine and Fluvoxamine were antagonised by Sertraline, thus supporting the belief that Sertraline is an antagonist of Sigma-1 receptors.
Fluoxetine is the only antidepressant so far that has been shown to directly elevate certain neurosteroids like Allopregnanolone, which itself has shown independent antidepressant & anxiolytic effects. Interestingly, Fluoxetine and its metabolite was able to raise this neurosteroid even at doses way below those used therapeutically.
Of all the SSRIs, Fluoxetine was the only one that was found to elevate Dopamine and Norepinephrine in the Prefrontal Cortex. This could be attributed to Fluoxetine's modest antagonist effect on the 5-HT2C receptor.
Fluoxetine has a really long half life (3-7 days approx), so if you skip a dose, you won't notice an immediate crash / rebound symptoms.
Only downside is that it inhibits a lot of enzymes that metabolise other drugs, so it has the potential to interact with a lot of other meds (by elevating their concentrations), so caution must be exercised if you plan to use any other drugs alongside Fluoxetine.
